I put the items from the agenda on the template in the same order and with a space to put the discussion and decisions/actions from the meeting.

To prepare a minute template simply use your agenda and put it in minute format. Under each agenda heading you can then record the appropriate information when you are in the meeting. Preparing your boss for meetings away from the office or at home

Follow up with the meeting organizer to request agenda and/or meeting materials, pre-reading, etc. If it is an external meeting, but local, enter location of meeting in calendar, provide your boss with address and directions, contact phone number, taxi chit.
