You can make the reservation directly through this site and the information gets forwarded over to the restaurant itself. T here is a great site called OpenTable.com , which allows you to book reservations online with particular restaurants all over the world. Then you receive an e-mail with the confirmation of all of this.

With my department, we purchase labels from another source and on the back of the label box shows the Avery type label with a 4 digit number.
